Accident Procedures

The Spencer County Board of Education is not liable for personal or property
damage as a result of vehicular accident or vandalism.

 

Student Injury

 

1. Provide First Aid or
2. Call 911 (477-5533 Dispatch)
3. Notify Parent
4. Student Accident Information Form Central Office Fax: Attn: Mark Thomas at 502 477-3259 (no cover sheet)
5. Doctor or Hospital Injury (if requested)
   School Authority: Please complete Section II of the K&K Claim Form (PDF - 82.5KB). Make a copy for the student file and give to parent along with completed Accident Report.
   Parent/Guardian: Please complete Section I of the K&K Claim Form (PDF - 82.5KB).
6. Parent will need to fax both reports to the given number in the claim form.

 

Student Athletic Injury

 

1. Provide First Aid or
2. Call 911 (477-5533 Dispatch)
3. Notify Parent

5. Doctor or Hospital Injury (if requested)

       2.  Submit Claim with private insurer

       3.  Voluntary Insurance Coverage is available
